For me, it will always be when Alistair pulled guard on a guillotine when he knocked down Stipe, instead of just trying to keep punching, get top position, and potentially finishing there.

The constant running away from Stipe, to eventually getting caught with a takedown with his backturned against Stipe while running was pretty bad too.

This was the first thing that came to mind. Also the Rumble/Cormier 2 when Rumble decided to wrestle.
