WASHINGTON — A senior congressional aide who has been briefed on the deaths of four U.S. servicemen in Niger says the ambush by militants stemmed in part from a "massive intelligence failure."
The Pentagon has said that 40 to 50 militants ambushed a 12-man U.S. force in Niger on Oct. 4, killing four and wounding two. The U.S. patrol was seen as routine and had been carried out nearly 30 times in the six months before the attack, the Pentagon has reported.

But it does make me think that the media is again ramping up the “body counts”. Four dead in Niger? This has been a major story for days. Did no soldiers die during Obama’s 8 years? Because I don’t know if I heard of any ...
And there in lies the problem. Routine? These guy are professionals they don't make mistakes like that on their own.

Obviously this was, at least “in part”, a failure of intelligence. The other parts of the failure may be more significant.

I concur with Tet68, either the US team and/or the Niger troops got complacent about the routine trips and didn’t plan for the ‘just in case something happens’.... “Intelligence Failure” would not be at the ‘national level’ because this was most probably a locally planned ambush and thus nothing for “intelligence” to find out about through various “intel assets.”
By definition all ambushes are intelligence failures. Maybe there was more that could be done, but it could just take a jihadi kid with a twitter account to send the nearest ISIS flash mob over if they think they have better numbers.
